Well assuming that "now" you know what went wrong, thats a good thing..
Lots of relationships ends while both the parties don't know exactly what went wrong, you know what I mean?

Since you know whats the problem, there's nothing better than sit with ur colleague & discuss with'em this situation. This might be hard in some cases, but at the end you'll feel better.
Good luck :)

nooni, I faced a similar situation with a dear colleague of mine couple of months ago. I knew she might reject the idea of us sitting together & discussing the conflict we were facing, but since (as I mentioned) she was close to me & I considered her more like a friend than a colleague, so I decided to take the initiative to fix things up, regardless of the outcome. So, its all up to you. At least you can say you gave it a try & (as we put it) "barrait thimitik".
btw, that colleague of mine didn't confess that she made a mistake nor did she appologize (although it was her fault), but I didn't care much... last thing I want is to have tention amongst my colleagues who I spend most of the day with`em.
